How It Works

Getting started is simple. Register to set up your account and gain access to your patient portal. This is where you will connect with your health team to schedule and view your appointments, chat, and access other resources and information you need, anytime, anywhere.

Primary Care FAQs

What if I already have a Primary Care Provider?

To access CloseKnit primary care services you must select CloseKnit as your primary care provider (PCP). If you already have a PCP you are happy with, we encourage you to continue seeing them. We're here for you if anything changes.

How far ahead can I schedule my primary care appointments?

Some visits, like a sick visit, may be available sooner than an annual preventive visit. Preventive visits are typically scheduled about three months in advance.

What if I need in-person care?

If you’re in need of in-person primary or specialty care, your Care Team will refer you to a local provider who specializes in the area of need. You may also be referred to see a CloseKnit provider in one of our physical locations, if available in your area.

What if I need specialty care?

If you're in need of specialty care, your Care Team will refer you to a provider (either in-person or virtual, as appropriate), who specializes in the area of need—such as heart conditions, joint care, etc.

What if I need lab work?

If lab work is needed before, or following, your visit, your provider will send an order to a facility near you.

Do you offer pediatric primary care?

We do not offer pediatric primary care at this time. CloseKnit primary care services are only available to adults (18+).

In which states is CloseKnit currently available?

In-person visits are available at One Post Office Square, 3rd Floor, Boston, MA 02109.

Virtual visits are available in most U.S. states and Washington, D.C. with coverage coming soon for NM. 

CloseKnit is not available outside of the US.
