Medical Records Requests

All CloseKnit patients can directly request assistance with medical records via chat in the app or by contacting Member Support at 866.233.6925, option 1, or via email at hello@closeknit.com.

If you are a patient joining us from AllCare, please contact our partner, MediCopy, to fulfill your medical records requests and assist with your Disability or FMLA paperwork. They can be reached Monday-Friday from 8:00am-4:00pm at the numbers listed below.
