Medical Records Requests

All CloseKnit patients can directly request assistance with medical records via chat in the app or by contacting Member Support at 866.233.6925, option 1, or via email at hello@closeknit.com.

If you are joining us from Comprehensive Primary Care or AllCare, please contact our partner, MediCopy, to fulfill your medical records requests and assist with your Disability or FMLA paperwork. They can be reached Monday-Friday from 8:00am-4:00pm at the numbers listed below.

Alternatively, you may complete this Records Release Out form. This completed form can be mailed, emailed, faxed, or brought to the front desk of the office to be processed. It may take up to 10 days for your request to be processed. There is a fee associated in providing copies of your records from our office. This fee is dependent on the number of copies requested.
