Frequently Asked Questions About Primary Care

Will I have to choose a Primary Care Provider to use Closeknit?

You do not need to choose a Primary Care Provider (PCP) at CloseKnit to use our clinical services. However, if you decide to choose us as your PCP, you will be paired up with a provider licensed in your area and will have full access to a dedicated Care Team, including free messaging.

What if I already have a Primary Care Provider?

If you already have a PCP, you can still access our clinical services (such as immediate care), and we can keep your PCP informed of the care we provide—if you’d like.

How far ahead can I schedule my primary care appointments?

Visits can be scheduled up to thirteen months in advance.

What if I need in-person care?

Patients are generally seen in our offices by appointment. We make all attempts to accommodate appointments with the provider of your choice as quickly as possible. Some offices offer walk-in and immediate care, as well as extended hours.

  • Please keep all appointments. If you need to cancel or change an appointment, please provide 24 hours’ notice.
  • Please tell our office about any changes in insurance and billing information.
  • Please arrive 15 minutes prior to your scheduled visit time.

What should I bring to my appointment?

Please bring the following to your appointment:

  • Insurance card and photo ID
  • Referral or physical forms
  • List of prescription and over the counter medications that you take regularly
  • Any radiological films and reports related to your visit
  • Co-payment or deductible payment

What if I need lab work?

If lab work is needed before, or following, your visit, your provider will arrange getting an order sent to a facility near you.
